Penn Medicine in Philadelphia is seeking an Emergency Department Social Worker to provide psycho social support, discharge planning, and care coordination across the ED.

The role collaborates with CM/SW teams, supports patients and families, and documents in medical records while engaging with community resources. A Master’s degree in social work is required; MSW/LSW preferred; 3+ years of behavioral health experience are helpful.

Qualifications

• Master of Arts or Science required.
• MSW or LSW preferred.
• 3+ years behavioral health experience in the field; healthcare setting experience helpful.

Responsabilités

• Coordinate discharge planning with CM/SW staff to ensure safe, timely transitions from hospital to home or community.
• Collaborate with the behavioral health care team to ensure continuity of care and support services for patients and families.
• Identify and address the psychological needs of patients and families.
• Provide counseling to patients, families and groups.
• Document in the medical record, statistics and reports as required.
• Maintain relationships with community agencies and stay updated on resources.
