Qualifications

• Master's degree required and independently licensed (LICSW or LMHC).
• Experience with pediatric patients, elders and families desirable.
• Proficient in medical terminology; EPIC and Outlook experience preferred.

Responsabilités

• Coordinate and document initial psychosocial assessments within 24 hours of admission/referral.
• Collaborate with RN Case Manager and care team to plan transitions home.
• Participate in interdisciplinary rounds and care conferences.
• Coordinate with APS/CPS and discharge planning with external agencies.
• Communicate psychosocial issues to care team and support patient/family needs.
