Job summary

Centene Corporation in Kansas is seeking a field-based Care Manager focused on populations with mental and behavioral health needs. The role travels approx. 50% and offers remote work the remaining time, with scheduling Monday–Friday and no weekends.

You will assess member needs, coordinate care plans, educate members and families, and liaise with providers to ensure services and benefits meet individual needs.

Qualifications

• Requires a Master's degree in Mental Health or Social Work or Nursing.
• 2–4 years of related experience.

Responsibilities

• Evaluates needs of complex/high risk members with mental/behavioral health needs and recommends a plan of care.
• Liaises between member/family, physician, and facilities/agencies.
• Supports members with mental/behavioral health needs (e.g., depression, bipolar, schizophrenia, PTSD, substance use).
• Performs home or site visits regularly to assess needs and coordinate with resources.
