Job summary

Qualifications

• Requires a Master's degree in Mental Health or Social Work or nursing accreditation.
• 2–4 years of related experience.
• Licensed Behavioral Health Professional or RN per state contract requirements (Sunflower plan).

Responsibilities

• Evaluate needs of the most complex and high-risk members and plan care.
• Act as liaison between member/family, physician, and facilities/agencies.
• Support members with mental/behavioral health needs and related conditions.
• Perform frequent home or site visits (monthly or more).
• Educate members and families on care options and benefits; provide MH-focused expertise.
• Coordinate community resources and service authorizations for services.
• Ensure referrals and access to providers and specialists.
• Maintain documentation in the electronic care management system; ensure compliance with guidelines.
• Provide leadership feedback to improve quality of care and service delivery.
• Perform other duties and ensure policy compliance.
• Comply with all policies and standards.

Education

Master's degree in Mental Health or Social Work

Graduate from Accredited School of Nursing
