Job summary

Centene Corporation seeks a Field-Based Care Management Specialist focused on mental and behavioral health to support SED waiver populations in Kansas. The role includes member advocacy, care planning, and collaboration with providers and community resources, with required travel to member sites.

Ideal candidates hold a Master's in Mental Health or Social Work (or Nursing with BH experience) and 2–4 years of related work; licensure as a BH professional or RN is required.

Qualifications

• Master's degree in Mental Health or Social Work, or Nursing with BH experience.
• 2–4 years of related experience.

Responsibilities

• Evaluates needs of complex, high-risk members with mental/behavioral health needs and recommends a plan of care.
• Acts as liaison and member advocate between member/family, physician, and facilities.
• Supports members with mental/behavioral health needs (e.g., depression, bipolar disorders, schizophrenia, PTSD, SUD).
• Performs home visits and site visits to assess member needs and coordinate resources.
• Educates members and families on services, benefits, and treatment options; coordinates community resources.
• Ensures referrals based on member needs and connects to providers and community resources.
• Maintains documentation in electronic care management system; complies with guidelines.
• Provides feedback to leadership on quality and efficiency improvements.

Education

Master's degree in Mental Health or Social Work

Graduate from an Accredited School of Nursing
