Job summary

Centene Corporation is seeking a skilled professional to develop, assess, and facilitate complex care management for behavioral health needs, with emphasis on education and personalized care plans for members and families.

The role supports foster care members across NW Kansas, with up to 75% local travel and remote work options. Monday–Friday schedule; Centene provides equipment and mileage reimbursement. Requires Master’s in Behavioral Health or related field and 2–4 years’ experience.

Qualifications

• Requires Master’s degree in Behavioral Health or Social Work or Nursing and 2–4 years of related experience.
• Licensed Master Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC) or RN with BH experience per state contract requirements.

Responsibilities

• Evaluates member needs via phone or in-home visits to develop care plans.
• Performs telephonic, digital, home, or other site visits to assess needs.
• Develops ongoing care plans for high-acuity members and coordinates providers and resources.
• Coordinates between member/family and care team to ensure access to services.
• Monitors plans and outcomes; provides recommendations based on member needs.
• Facilitates care coordination to ensure timely access to care.
• Collects and maintains member information for regulatory compliance.
• Provides education to members and families on procedures and benefits.
• Provides feedback to leadership on improving care delivery.
• Performs other duties and complies with policies.
