Job summary

Centene is seeking a field-based Care Manager in Kansas to develop and coordinate holistic care plans for members with mental/behavioral health needs. The role travels throughout Kansas and involves educating families on available services and benefits.

Ideal candidates hold a Master’s in Mental Health or Social Work (or nursing) and have 2–4 years of related experience. Travel is up to 50% with a standard M-F schedule.

Qualifications

• Requires a Master’s degree in Mental Health or Social Work or nursing with 2–4 years of related experience.
• Licensed Behavioral Health Professional or RN may be required for Sunflower plan depending on state contract.
• Pay range provided is hourly and based on location and eligibility.

Responsibilities

• Evaluates member service needs and develops care/service plans and educates members and families on available services.
• Assesses high-risk members with mental/behavioral health needs and recommends a plan of care.
• Acts as liaison between member/family, physician, and facilities/agencies.
• Performs home or site visits monthly or more to assess needs and collaborate with resources.
