Centene Corporation in Kansas is seeking a Behavioral Health Care Manager for field-based operations supporting individuals on the SED waiver program. The role involves assessing complex mental/behavioral health needs, coordinating care plans, and educating members and families.

Travel about 50% is required, with remote work the rest of the time; weekdays only; Marion County candidates preferred. Responsibilities include liaison between members, families, physicians, and facilities; home visits;

Formación

• Requires a Master's degree in Mental Health or Social Work or graduate from an accredited School of Nursing.
• 2–4 years of related experience.
• Licensed Behavioral Health Professional or RN per state contract requirements.

Responsabilidades

• Evaluates the needs of high-risk mental/behavioral health members and recommends a plan of care.
• Acts as liaison between member/family, physician, and facilities/agencies.
• Supports members with mental/behavioral health needs and complex conditions.
• Performs recurring home or site visits to assess needs and coordinate resources.
• Educates members and families on preventive care, treatment options, and benefits.
• Educates on community resources and coordinates service authorization.
• Ensures referrals and access to providers and services.
• Maintains documentation and ensures compliance with guidelines.
• Provides leadership feedback to improve quality of care.
• Performs other duties as assigned.
• Complies with policies and standards.
