Qualifications

Responsibilities

• Interviews patients to assess functional level and completes psychosocial screenings.
• Develops a plan of care and discharge plans with the interdisciplinary team.
• Provides crisis intervention and uses de-escalation techniques.
• Evaluates social determinants of health to improve quality of care.
• Coordinates referrals and communicates with patients, families and care team.
• Provides counseling regarding coping and illness management.
• Maintains privacy and security of confidential information.
• Documents all work timely and in accordance with regulatory standards.
