Job summary

Garrett Regional Medical Center invites a dedicated social worker to join the Behavioral Health team. The role focuses on psychosocial assessments, discharge planning, and linkage to community resources to support patients and families.

You will work with a multidisciplinary team to develop treatment plans and document care decisions. Required bachelor’s in social work or related field, BLS within 30 days, and AVADE training within 30 days.

Qualifications

• Bachelor’s degree in social work, psychology, sociology or related field.
• BLS certification must be obtained within 30 days of hire.
• AVADE training within 30 days of hire.

Responsibilities

• Perform intake assessments and evaluate clients’ psychosocial needs.
• Participate in multidisciplinary treatment planning with attention to individual differences.
• Assess discharge planning needs and coordinate appropriate placements.
• Maintain documentation in the medical record of evaluations and plans of care.
• Provide counseling on end-of-life decisions and advance directives.
