Job summary

Qualifications

• Master's degree in Social Work from an accredited college or university; field education/practicum completed.
• Two or more years’ experience working with patient population preferred.

Responsibilities

• Conduct psychosocial assessment and develop an effective treatment plan with patient safety as a priority.
• Coordinate patient cases and discharge planning with input from the multidisciplinary team.
• Provide crisis intervention and educate families about implications of life situations.
• Facilitate education, process groups, support groups and referrals as requested.
• Maintain accurate patient documentation and ensure records comply with facility standards.
