Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management services and short-term psychotherapy to patients with behavioral health needs. This role works with Care Teams across multiple clinics, coordinating with a psychiatric consultant to develop care plans and referrals.

Responsibilities include screening, assessment, and evidence-based treatment delivery, with possible remote work days based on program metrics.

Qualifications

• Master’s degree in mental health counseling or related field from an accredited program.
• Highest clinical level of license (or local equivalent).
• Experience with screening for mental health and/or substance use disorders.
• Experience with assessment and treatment planning for mental health and/or substance use disorders.
• Familiarity with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation, Problem Solving, Cognitive Behavioral therapy).
• US work authorization is required.

Responsibilities

• Collaborate with primary care teams to identify patients for the behavioral health program.
• Develop care plans and coordinate referrals to psychiatric consultants and external resources.
• Provide or facilitate short-term counseling and evidence-based treatments.
• Coordinate care with psychiatric consultants, including warm handoffs for telehealth visits.
• Maintain accurate documentation in the EMR and communicate changes in care plans.
