Job summary

Oak Street Health is seeking a Behavioral Health Specialist to provide care management and short-term psychotherapy within our primary care clinics. You will coordinate with a psychiatric consultant and other care teams, assess patient needs, make referrals, and support program-wide initiatives.

Qualifications

• Master’s degree in mental health counseling or related field from an accredited program.
• Licensed at the highest clinical level (LCSW/LPC/LMFT or local equivalent).
• Experience with screening for common mental health or substance use disorders.
• Experience with assessment and treatment planning for common mental health disorders.
• Familiarity with brief, structured intervention techniques (e.g., MI, CBT).
• US work authorization.

Responsibilities

• Collaborate with primary care teams to refer patients to the Behavioral Health Program.
• Develop care plans and coordinate referrals to psychiatric consultants or external resources.
• Provide short-term counseling and evidence-based treatments.
• Coordinate care, including warm handoffs for telehealth and care-planning meetings.
• Document patient encounters accurately in the EMR and participate in program initiatives.
