Job summary

Advocate Health's Davidson Behavioral Health: Social Services department seeks a master's-level clinician to provide counseling and case management within a collaborative team. You will assess patient needs, develop care plans, and coordinate discharge with other providers, while maintaining timely documentation.

You will work a 40-hour week, with M-F 1st shift, and must complete CPI and CPR training within 90 days. Total rewards and benefits are provided.

Qualifications

• Master's degree in a relevant human services field is required.
• Must be issued an associate licensure and/or certification within 6 months of employment.
• Successful completion of CPI and CPR training required within 90 days of hire.
• Clinical experience preferred; case management with SMI or substance abuse preferred.
• Good therapeutic communication skills.

Responsibilities

• Collaborates with guardians and community agencies to meet patient care needs.
• Assesses patients’ problems, needs, and level of functioning.
• Writes and implements treatment plans and discharge plans.
• Provides individual, group, and family counseling; evaluates mental health conditions.
• Maintains accurate documentation in the medical record and presents cases in team meetings.
• Coordinates discharge planning with the next provider.

Education
