Qualifications

• Master's degree in a relevant human services field required.
• Must obtain associate licensure/certification within 6 months of employment.
• CPI and CPR training required within 90 days of hire.
• Clinical experience preferred; case management experience with SMI and/or substance abuse preferred.
• Good therapeutic communication skills.

Responsibilities

• Collaborate with guardians and community agencies to meet patient care needs.
• Assess problems, needs and functioning to write treatment plans.
• Provide psychotherapy and human development-based interventions.
• Conduct discharge planning and plan of care with next provider.
• Document patient care accurately in medical records.
• Present cases in clinical team meetings.
