Job summary

Advocate Health is seeking a licensed mental health professional to provide comprehensive patient care within our Behavioral Health team. Responsibilities include assessment, treatment planning, individual and group counseling, and discharge planning, with emphasis on evidence-based practices and documentation accuracy.

The role requires a master's degree in a relevant field, state licensure, and completion of CPI and CPR within the first 90 days.

Qualifications

• Master's degree in a relevant human services field is required.
• Associate licensure and/or certification in the state of employment within 6 months is required.
• Successful completion of CPI and CPR training within 90 days of hire.

Responsibilities

• Collaborate with guardians and community agencies to meet patient care needs.
• Assess patients' problems, needs, and level of functioning.
• Develop and write treatment plans aligned with diagnosis, age, and clinical presentation.
• Provide multiple modalities of treatment based on current clinical standards.
• Counsel individuals, groups, and families and document outcomes in the medical record.
• Conduct discharge planning and coordinate plan of care with the next provider.
