Job summary

Advocate Health is seeking a full-time Behavioral Health Social Services professional in Davidson to provide counseling, assessment, treatment planning, and discharge coordination. Requires a Master’s in a relevant field and timely licensure; CPI/CPR training within 90 days is expected.

Qualifications

• Master's Degree in a relevant human services field (Social Work, Counseling, Psychology or related).
• Must obtain associate licensure/certification within 6 months of employment.
• Successful completion of CPI and CPR training within 90 days of hire.
• Clinical experience preferred; case management with Serious and Persistent Mental Illness population or substance abuse experience is a plus.
• Excellent therapeutic communication skills and ability to document care accurately.

Responsibilities

• Utilizes resources to meet patient care needs by collaborating with guardians and community agencies.
• Assesses patients’ problems, needs, and level of functioning.
• Writes treatment plans according to diagnosis, age, and clinical presentation.
• Provides various treatment modalities based on current clinical standards.
• Assists individuals, groups, and families through the counseling relationship and treats mental disorders.
• Writes discharge plans and follows through with termination process.
• Delivers patient care per policies and standards; documents care in medical records.
• Designed and implements patient education; coordinates discharge planning with next provider.
• Presents cases at clinical team meetings in a clear, concise manner.
