Responsibilities

• Collaborate with guardians and community agencies to meet patient care needs.
• Assess patients' problems, needs, and functioning level.
• Develop and write treatment plans based on diagnosis and client age.
• Provide psychotherapy modalities per current clinical standards.
• Document care accurately in the medical record.
• Coordinate discharge planning and follow-up with next provider.
