Job summary

Premier Health Partners, in partnership with Samaritan Behavioral Health Inc. and The Lincoln Center in Troy, is seeking a Part-Time Case Manager to support planning and coordination of treatment services for children and adolescents.

The role involves coordinating services across agencies, maintaining CareLogic records, and advocating for clients in various settings. Requires a Bachelor’s degree and 1–3 years in mental health, with LSW preferred.

Qualifications

• Two year degree in social science field or equivalent education and experience.
• Minimum of 1-3 years of job-related experience in mental health or related field.
• Knowledge of DSM-5 or subsequent revisions relevant to coordination of services.
• LSW preferred.

Responsibilities

• Adhere to PHP/SBHI Patient Experience philosophy, guidelines and tools.
• Coordinate care with referral sources, internal teams and external providers.
• Complete direct service paperwork within agency timelines.
• Provide direct, face-to-face services to clients via individual, group, and family therapies.
• Provide CPST services to coordinate clinical care through meetings, calls and reports.
• Maintain schedules and caseload in CareLogic according to standards.
• Communicate with medical providers to coordinate client care.
• Demonstrate strong customer service in clinical care delivery.
• Maintain medical records in line with regulatory standards and documentation.
• Apply IT and quality improvement policies to documentation and records.
