Banner Health is seeking a PRN Behavioral Health Case Manager to provide comprehensive care coordination across adult and adolescent units, with potential to float between locations and programs. The role includes assessment, planning, and linkage to services in outpatient and intensive programs, ensuring safe, patient-centered care in a scalable health system.

Responsibilities include coordinating discharge, collaborating with physicians and payers, and supporting an integrated care approach

Qualifications

• Master’s degree in social work or related field; 5 years clinical experience.
• Licensed in social work, professional counseling, or marriage and family.
• Knowledge of care management and working with interdisciplinary teams.
• Arizona Fingerprint Clearance Card and related background checks as required.

Responsibilities

• Manage patients across the health care continuum to achieve optimal clinical and satisfaction outcomes.
• Provide assessment, psychoeducation, and psychotherapy to individuals, families, and groups; coordinate care from admission to discharge.
• Lead process-improvement activities for populations of patients and manage discharge planning with the health care team.
• Evaluate medical necessity and optimize patient outcomes; document communications with payers and regulators.
• Educate internal team members on case-management concepts and integrate them into daily practice.
