Job summary

Primary Health Solutions is seeking a School-Based Case Manager to deliver community psychiatric support to youth and families in a full-time 40-hour work week. You will collaborate with a Program Manager and a larger team to develop mutual treatment plans and support clients in achieving goals.

The role requires foundation in human services, a valid driver's license, and experience working with families and children.

Qualifications

• Degree in human services required; experience with families and children preferred.
• Valid driver’s license and access to a vehicle are required.
• Ability to document and maintain clinical records and case notes.

Responsibilities

• Establish rapport with youth and families and assess treatment needs.
• Provide mental health interventions to help clients reach goals with monthly face-to-face contact.
• Coordinate ongoing assessments of needs and self-management support for clients and families.
• Provide crisis intervention for clients in acute distress.
• Educate families on accessing and utilizing community resources.
• Maintain documentation in the Integrated Clinical record and meet contract timelines.
• Collaborate with other providers to ensure comprehensive, client-centered care.
