Job summary

Crusader Community Health is seeking a Behavioral Health Case Manager to deliver comprehensive case management and counseling in clinic and community settings. You will work within the Integrated Primary Care model with children, adolescents, and adults, coordinating with providers, families, and community services.

The role reports to the Chair of Behavioral Health and involves education, advocacy, and care coordination across a network of partners in the Rock River Valley region.

Qualifications

• Bachelor’s degree in a human services field is required.
• Master’s degree in Social Work, Counseling, Psychology or related field preferred.
• CADC certification or successful completion of CADC is required/preferred.

Responsibilities

• Provide comprehensive case management and behavioral health counseling.
• Educate patients and families; train and support skills.
• Coordinate linkage, referrals, and advocacy with community services and agencies.
• Collaborate with providers, patient, family, and managed care organizations.
