Job summary

Summit Pointe is seeking a dedicated Case Manager to support youth with emotional disturbances in the Battle Creek, MI area. You will complete psychosocial assessments, plan individualized goals, and coordinate connections to community supports including Outpatient, Wraparound, and Youth Peer Services.

You will ensure safety planning, maintain compliant records, and participate in ongoing training and credentialing aligned with licensure requirements.

Qualifications

  • Bachelor’s degree in a human services field is required.
  • Licensed Social Work degree preferred; Michigan licensure preferred.
  • One year of computer experience with Word/Excel and EMR software preferred.

Responsibilities

  • Completes psychosocial assessments with diagnosis and care recommendations.
  • Engages in person-centered planning to develop goals and objectives.
  • Performs risk assessments and safety planning with youth and families.
  • Coordinates referrals to Outpatient, Wraparound, and Youth Peer Support Services.
  • Maintains records per agency and funding source regulations.
  • Commits to competency improvement and ongoing professional development.

Job description

Job Summary/Objective:Case Management services are a community-based service within the youth services continuum. Case Management services assist youth and families who live with severe emotional disturbances and/or cognitive and/or developmental delays. The role of a case manager is to link youth and families to necessary supports in our community and monitor ongoing needs and progress toward individualized goalsEssential Duties and Functions:Completes psychosocial assessments inclusive of a diagnosis, case conceptualization, level of care assessment, and recommendations for treatment.Engages in person-centered planning to develop goals and objectives.Engages in risk assessment and completes safety planning with youth and families.Coordinates appropriate referrals to other youth-oriented programs such as Outpatient, Wraparound, and/or Youth Peer Support Services.Performs outreach prevention activities within the community.Maintains record compliance according to agency guidelines and standards as well as funding source regulations.Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirements or minimum of 24 hours per year of clinically based trainings), implementation of evidence based practices, peer review attendance, and professional improvement practices.QualificationsBachelor’s degree in a human services field is required.Limited or Full Licensed Bachelor’s in Social Work is preferred.Child Mental Health Professional, as defined by the Michigan Department of Health and Human Services provider qualification, is preferred.One year of computer experience – Word, Excel, and other electronic medical record software preferred.Benefits:Premium health and vision within 30 days of hireCompany sponsored Health Reimbursement AccountCompany paid short and long-term disabilityEmployee assistance program401(K) match up to 5% after one year of serviceSocial security alternative contributionsFree YMCA membershipGenerous paid time off14 paid holidaysContinuing education reimbursementLicensure study materials and exam reimbursementTrauma Informed System of Care:A trauma-informed approach to service delivery emphasizes physical and emotional safety, choice, empowerment, and trustworthiness for customers.
