CVS Health is seeking a Case Manager to coordinate care for dual eligible populations, blending clinical and social support to meet complex health and social needs. You will assess, plan, coordinate, and advocate for patients, collaborating with medical teams and community resources.

This full-time role requires IL residency, strong communication, and the ability to travel up to 50-75% as needed. A Master’s in Behavioral/Mental Health and appropriate licensure are preferred.

Qualifications

• Master's degree required or equivalent in behavioral/mental health field.
• Licensed in IL as LCSW or LCPC preferred or required.
• Strong analytical and communication skills for care coordination.

Responsabilités

• Assess members' needs and eligibility; plan and coordinate care.
• Provide crisis intervention and refer to appropriate clinical providers.
• Apply clinical guidelines to ensure appropriate benefits administration.
• Collaborate with supervisors and Medical Directors to optimize outcomes.
• Monitor and document plans of care with the care team.
• Escalate quality of care issues through proper channels.
