Qualifications

• Bachelor's degree or higher in Behavioral or Social Science or related field.
• Six months' experience in case management, including planning and evaluating options and services.
• Six months' experience in clinical treatment, counseling or eligibility determination as part of a team.
• Six months' experience interpreting laws, rules, and procedures.
• Six months' experience in narrative report writing.

Responsibilities

• Interviews clients and family members to obtain complete case history.
• Provides guidance to the client's family to enable solutions and positive interactions.
• Evaluates information and refers to proper review channels.
• Prepares narrative, statistical, and court reports as necessary.
• Participates in a multi-disciplinary team for assessment and treatment planning.
• Prepares forms for services or benefits and coordinates referrals.
• May appear at court proceedings to testify on client issues.
• Provides guidance to organizations and staff on findings related to client care.
