Job summary

Anew Behavioral Health LLC is seeking a dedicated Behavioral Health Case Manager to coordinate care for clients facing mental health challenges. The role involves assessing client needs, developing individualized service plans, and collaborating with multidisciplinary teams to promote recovery and client-centered services.

The Case Manager will advocate for clients in treatment decisions, manage service plans, and ensure documentation and billing align with CARF and CMS standards.

Qualifications

• Strong interpersonal skills and the ability to communicate effectively with clients, families, and professionals.
• Knowledge of behavioral health practices and community resources.
• Ability to work independently and as part of a team.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and client family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving personal independence and assist in developing daily living skills.
• Perform other duties as assigned.
• Upholds Documentation and Billing practices associated with CARF, CMS, and as training indicates.
