Job summary

Community Bridges, Inc. (CBI) is seeking a Familiar Face Case Manager to coordinate care and discharge planning for individuals utilizing crisis services. The role focuses on preparing patients for clinically appropriate services and coordinating with all involved parties to reduce barriers to treatment.

The position reports to the Clinical Director. Requires a high school diploma or GED; an associate's degree is preferred.

Qualifications

• High school diploma or GED required; associates degree preferred.
• Six+ months experience across multiple settings in mental health and substance use.
• One+ year experience preferred.
• At least six months recovery from substance use or mental health disorders preferred.
• Current Driver’s License in good standing.
• 39-month Motor Vehicle Record (MVR).
• Behavioral Health Technician (BHT) per state policies.

Responsibilities

• Coordinate care and discharge planning for crisis service users.
• Prepare patients for the most clinically appropriate services.
• Coordinate care with involved parties and reduce barriers to treatment.
