Responsibilities

• Communicate with health care teams regarding assessments and discharges.
• Provide guidance on benefits, reimbursement guidelines, and regulatory compliance.
• Complete initial psychosocial assessments and develop safety plans for community living.
• Facilitate discharge planning with multidisciplinary teams to ensure safe transitions.
