Duke Regional Hospital in Durham, NC seeks an experienced social worker to manage a designated caseload, coordinate timely assessments, plan and evaluate discharge needs, and ensure efficient care transitions across the continuum.

Qualified candidates will hold an MSW and be licensed as an LCSW or eligible. Typical duties include collaboration with hospital teams and payers, crisis assessments, and documentation adherence. This full-time role is on-site.

Qualifications

• Master's degree in social work (MSW) from an accredited program.
• Current licensure as an LCSW or eligibility for licensure in NC.
• 3+ years of adult/mental health social work experience.

Responsabilités

• Coordinate and complete discharge planning across care transitions.
• Communicate with health care team on assessments and orders.
• Document assessments and discharge plans per guidelines.
• Collaborate with Utilization Management and payers to optimize care.
• Perform crisis assessments and coordinate psychiatric follow-up.
