Job summary

Duke Raleigh Hospital, A Campus of Duke University Hospital, seeks a Case Manager to coordinate discharge planning and care transitions across the continuum of care. You will assess patient needs, develop and evaluate discharge plans, and coordinate with teams, patients, families, and partners for safe, efficient transitions.

Requirements include BSN or MSW, a minimum of 3 years' experience, and Case Management Certification within 3 years of hire.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification within 3 years of hire.
• For BSN-prepared candidates, NC RN license required.

Responsibilities

• Assess patients for case management needs including discharge planning and care transitions.
• Develop, implement, and evaluate individualized discharge plans.
• Coordinate services (HHA, DME, SNF, rehab) and follow-up.
• Maintain timely documentation and ensure regulatory compliance.
• Collaborate with care teams and families; provide patient education.
• Participate in QAPI activities and after-hours coverage as needed.
