Job summary

Qualifications

• Bachelor's degree in a behavioral health area or related field preferred.
• One or more years of relevant Behavioral Health experience preferred.
• Experience in case management is desirable.

Responsibilities

• Develop and implement individualized service plans (ISPs) for patients.
• Coordinate discharge planning and aftercare services.
• Liaise with referral sources, providers, and families to ensure continuity of care.
• Document clinical activities and coordinate care across teams.
• Coordinate eligibility screening for entitlements (AHCCCS, SSI/SSD, etc.).
• Initiate transition planning at admission and monitor progress.
• Schedule aftercare appointments and coordinate community resources.
