Peak Behavioral Health in Santa Teresa, NM is seeking a Case Manager - Acute to support the clinical services team. You will develop and implement ISP-based activities, coordinate discharge planning, and connect patients with aftercare resources while collaborating with families and providers.

• Bachelor's degree preferred in behavioral health, marketing, business administration or related field.
• 1+ year of related experience in Behavioral Health preferred.
• Previous case management experience preferred.

Responsabilidades

• Develop and implement the service plan (ISP) for patients.
• Coordinate discharge planning and aftercare connections.
• Collaborate with family and providers to implement aftercare options.
• Document clinical activities and coordinate care across teams.
• Arrange community resources to support discharge and transitions.
• Provide crisis intervention as needed.
