Job summary

Peak View Behavioral Health, located onsite in the United States, is seeking a Case Manager to support clinical services by coordinating discharge and aftercare planning, and connecting patients with needed resources. You will maintain records and collaborate with the treatment team to implement the individualized service plan.

Qualifications include a Bachelor’s in a behavioral health field, 1+ year in behavioral health, and CPR/de-escalation certification.

Qualifications

• Bachelor’s Degree in a behavioral health area, marketing, business administration or related field preferred.
• One or more years previous related experience in Behavioral Health preferred.
• Previous experience in case management preferred.
• CPR and de-escalation and restraint certification required (training available upon hire). First aid may be required based on state or facility requirements.

Responsibilities

• Participates in the development of the service plan.
• Implements appropriate activities and programs per ISP and scope of practice.
• Coordinates discharge planning and aftercare options with the team.
• Maintains patient clinical records and service documentation.
• Coordinates referrals to mental health, community services, housing, and employment.
