Job summary

Peak Behavioral Health in Santa Teresa, NM seeks a Case Manager to support clinical services by coordinating case management across units, discharge planning, and aftercare placement. The position requires a behavioral health degree and prior case management or related experience, with CPR/de-escalation certification.

Qualifications

• Bachelor’s degree or higher in a behavioral health field or related area.
• 1+ year of related behavioral health experience preferred.
• Experience in case management preferred.
• CPR and de-escalation/restraint certification required.

Responsibilities

• Participates in development of the service plan.
• Implements activities and programs.
• Implements services per ISP within case manager's scope.
• Coordinates discharge and aftercare planning with referrals and providers.
• Maintains comprehensive clinical records and service delivery documentation.
• Coordinates releases, records, referrals and connections to psychiatric, therapy, housing, employment services.
• Collaborates with treatment team to engage and coordinate care for patients and families.
• Ensures referrals for services on the service plan are made and coordinated.
• Assists with financial entitlements screening and AHCCCS applications.
• Ensures discharge planning begins at admission.
• Facilitates communication with referral sources and providers.
• Arranges community resources to support discharge and aftercare.
• Supports continuum of care with documentation to providers.
• Employs crisis intervention as needed.
• Schedules aftercare with PCPs/psychiatrists/therapy programs.

Education

Bachelor’s Degree in a behavioral health area, marketing, business administration or related field

CPR and de-escalation and restraint certification

One or more years experience in Behavioral Health

Previous experience in case management
