Job summary

Park Place Behavioral Health Care seeks a dedicated case manager to provide comprehensive services for individuals committed to a State Mental Health Treatment Facility. You will coordinate psychosocial assessments, recovery plans, and ongoing community supports to promote engagement and stable housing.

Responsibilities include discharge planning, linkage to benefits, and ongoing collaboration with SMHTF staff and local providers.

Qualifications

• Bachelor's degree in Social Work, Sociology, Psychology, or related human services field required.
• Master’s degree preferred.
• Minimum two years of experience in a behavioral health environment.

Responsibilities

• Work with SMHTF to develop the psychosocial assessment and recovery plan.
• Participate in monthly reviews of the recovery plan.
• Assist in discharge planning and develop community-linked services.
• Maintain monthly contact with SMHTF staff and with the client’s family as appropriate.
• Coordinate Social Security office visits to activate benefits.
• Maintain progress notes in the client record.
• Develop and maintain relationships with local behavioral health providers and housing facilities.
