Job summary

Community Bridges, Inc. (CBI) is seeking a Case Manager to provide comprehensive case management in community and/or office settings.

You will assist patients and families with planning, connecting, outreaching, advocating, coordinating, and monitoring needs, while helping access health services, financial assistance, employment, education, social services, and natural supports. You will assess needs, develop and monitor individualized treatment plans, and document services accurately,

Qualifications

• High school diploma or GED required; associates degree or higher in behavioral health preferred.
• Minimum six months of recovery from substance use and/or mental health disorders preferred.
• 1–3 years of experience working with the GMH/SU population required.
• 1–3 years of experience in outreach and engagement activities required.
• 1–3 years of experience in a medical setting preferred.
• Valid identification required.
• Arizona Driver's License (preferred).
• 39-month MVR preferred.
• Behavioral Health Technician (BHT) per policy
• Arizona Fingerprint Clearance card.

Responsibilities

• Provide case management in community and/or office settings to patients and families.
• Assist in planning, connecting, outreaching, advocating, coordinating, and monitoring needs and progress.
• Help patients access health services, financial assistance, employment, education, social services, and natural supports.
• Assess needs, develop and monitor individualized treatment plans, and document services timely.
• Promote recovery philosophy and a safe recovery environment for clients.
