(CBI) seeks a Specialty Case Manager to provide community-based case management for patients with serious mental illness, coordinating health services, housing, employment and education supports. The role emphasizes access, planning, advocacy, and recovery-oriented practice.

The candidate will assess needs, develop individualized plans, document services, and promote a safe recovery environment for high-risk clients in Arizona.

Qualifications

• High school diploma or GED required.
• Associates Degree or higher in a field related to behavioral health preferred.
• 1 year of experience in a behavioral health position providing outreach and engagement activities.

Responsabilités

• Provide community-based case management to patients with serious mental illness.
• Assist in planning, connecting, advocacy, coordination and monitoring of health services.
