Job summary

Community Bridges Inc. is seeking a Case Manager to provide comprehensive case management in community and/or office settings. You will assist patients and families in planning, connecting resources, outreaching, advocating, coordinating and monitoring needs and progress.

You will help patients access health services, financial assistance, employment, education, social services and supports, while documenting services and supporting recovery with a safe environment for clients.

Qualifications

• High school diploma or GED required; associate degree or higher in behavioral health preferred.
• Minimum six months recovery from substance use and/or mental health disorders preferred.
• 1–3 years experience with GMH/SU population required.
• 1–3 years experience in behavioral health outreach and engagement required.
• 1–3 years experience in medical settings preferred.
• Valid identification required.
• Arizona driver license preferred; 39-month MVR preferred.
• BHT per state policy and CBI procedures; AZ Fingerprint Clearance card.

Responsibilities

• Provide case management services in community and/or office settings.
• Assist patients and care team in planning, connecting, outreaching, advocating, coordinating and monitoring needs and progress.
• Help patients gain access to health services, financial assistance, employment, education, social services and supports.
• Assess needs, develop and monitor individualized treatment plans, document services timely and accurately.
• Promote recovery and a safe environment for clients.
