Job summary

Community Bridges, Inc. (CBI) seeks a Familiar Face Case Manager to coordinate care and discharge planning for individuals using crisis services at high utilization rates.

You will prepare patients for appropriate services and help navigate arrangements with involved parties, reporting to the Clinical Director. The role requires a background in behavioral health, a valid driver’s license, and experience across mental health and substance use settings.

Qualifications

• High school diploma or GED required; associate degree in behavioral health preferred.
• Minimum of six months of experience in multiple settings and levels of care, including mental health and substance use.
• One or more years of experience preferred.
• Minimum of six months of recovery from substance use and/or mental health disorders preferred.
• Current Driver’s License in good standing.
• 39-month Motor Vehicle Record (MVR) required.
• Behavioral Health Technician (BHT) per policy.

Responsibilities

• Coordinate care and discharge planning for crisis service patients.
• Assist patients in obtaining clinically appropriate services; reduce barriers to treatment.
• Collaborate with clinicians and involved parties to optimize patient outcomes.
