Job summary

Community Bridges, Inc. seeks a Specialty Case Manager to provide community-based case management for individuals with serious mental illness or high-risk acuity. The role involves planning, advocacy and coordination to help clients access health services, housing, employment, education and social supports.

Qualifications

• Highschool diploma or GED required.
• Associates Degree or higher in a field related to behavioral health preferred.
• 1 year of experience in a behavioral health position providing outreach and engagement activities.
• Current AZ Driver's License and 39-month MVR.
• Peer Certification (if applicable)
• Arizona Fingerprint Clearance card
• Jail Clearance (if applicable)

Responsibilities

• Provide case management services in the community to patients experiencing serious mental illness.
• Assist in planning, advocacy, coordination and monitoring to access health services, housing, employment and social supports.
• Develop and monitor individualized treatment plans and document services accurately.
• Support recovery-oriented care and safe recovery environments.
• Implement advanced techniques to serve high-risk individuals in associated program.
