Job summary

LHH in Phoenix, AZ is seeking a Behavioral Health Case Manager to provide direct client engagement, support services, and behavioral health interventions for adults with Serious Mental Illness (SMI). This onsite role emphasizes treatment plan support and recovery goals rather than a large caseload.

You will conduct wellness check-ins, coordinate with clinical teams, and help clients access community resources while maintaining thorough documentation.

Qualifications

• Experience in behavioral health, BHT, peer support, or case management preferred.
• Experience with Serious Mental Illness (SMI) populations strongly preferred.
• Strong communication and de-escalation skills.
• Certification in behavioral health is a plus.

Responsibilities

• Provide behavioral health support and client engagement services.
• Assist clients with accessing community resources.
• Support treatment plans and recovery goals.
• Conduct wellness and support check-ins.
• Assist with crisis prevention and stabilization efforts.
• Collaborate with clinical and behavioral health teams.
• Maintain accurate and timely documentation.
