Job summary

Resilient Health in Gilbert, AZ is seeking a Case Manager to coordinate care for members with mental illness and/or addiction. You will assess needs, develop individualized plans and facilitate community and home-based services to support recovery.

You will maintain charting and ensure timely, compliant documentation, while coordinating with teams, families and providers to secure a wide range of services and supports.

Qualifications

• High School Diploma or equivalent with six months of behavioral health work experience or internship.
• Associates Degree with three months of behavioral health experience or internship.
• BHT Certificate, or Bachelor's/Master's Degree as applicable.

Responsibilities

• Participates in the assessment, evaluation and planning of patient care.
• Develops individualized service plans for participants.
• Provides community and home-based services to support treatment goals.
• Coordinates meetings, staffing, case conferences and other meetings as appropriate.
• Provides referrals to outside providers and assists in securing social services.
• Maintains documentation (charting) per agency and funding source requirements.
• Communicates with participants or families to coordinate consent, releases and referrals for care.
• Coordinates transportation to and from appointments when appropriate.
• Meets with service providers to review utilization and progress toward goals.
• Provides participant advocacy and education as needed.
• Develops transitional service plans prior to reentry into the community.
