Job summary

COPE Community Services, Inc. is seeking a Behavioral Health Case Manager (BHCM) in Tucson to serve as a primary contact and advocate for members navigating recovery.

You will deliver face-to-face, phone, virtual, and in-home services as needed, and you will develop and update person-centered service plans promoting member participation. You will monitor progress, document outcomes, and coordinate with providers, probation, and other involved service teams to ensure comprehensive care and timely

Qualifications

  • Must have a Bachelor's or master's degree.
  • Associate degree with 1 year behavioral health experience.
  • High School Diploma with Peer Support Specialist cert plus 2 years BH experience.
  • High School Diploma with Medical Assistant cert plus 2 years BH experience.
  • High School Diploma with 3 years BH experience.

Responsibilities

  • Serve as a point of contact and advocate for members.
  • Develop, implement, and update person-centered service plans.
  • Monitor progress and document progress on plans and goals.
  • Coordinate communication with primary care providers and other involved services.
  • Ensure ongoing collaboration and documentation of covered services.

They provide face-to-face, phone, virtual, and in-home services as needed by each member's journey to recovery. They develop, implement, and update person-centered individual service plans (promoting member participation) in a timely manner. Monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting the progress on the service plan as defined by policies and procedures. An unacceptable driving record includes (but is not limited to) the following:more than two [2] moving violations in the last three [3] years or;more than one [1] at-fault accident within the last three [3] years or;any major violation (i.e. DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years.Other driving related violations may be considered unacceptable. At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.Applicants must be able to pass a pre-employment drug screen.This is a Safety Sensitive Position.
