COPE Community Services, Inc. is hiring a Behavioral Health Case Manager in Tucson, AZ. The role delivers in-person, phone, virtual, and in-home services to support members on their recovery journey.

You will develop and update person-centered service plans and monitor progress, coordinating with providers and other stakeholders to ensure integrated care. A mix of qualifications is accepted, including degrees with relevant experience.

Qualifications

• Must be able to pass pre-employment drug screen and background check.
• Position requires 21+ years of age with AZ driver's license and clean driving record.
• Experience in coordinating services and documenting progress preferred.

Responsibilities

• Provide face-to-face, phone, virtual and in-home behavioral health services as needed.
• Develop, implement, and update person-centered service plans promptly.
• Monitor progress and coordinate with primary care providers and other service partners.
• Maintain timely documentation and communication per policy.

Education

Bachelor's or Master's degree

Associate degree + 1 year behavioral health experience

HS Diploma + Peer Support Specialist cert + 2 years behavioral health

They provide face-to-face, phone, virtual, and in-home services as needed by each member's journey to recovery. They develop, implement, and update person-centered individual service plans (promoting member participation) in a timely manner. Monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting the progress on the service plan as defined by policies and procedures. An unacceptable driving record includes (but is not limited to) the following:* more than two [2] moving violations in the last three [3] years or;* more than one [1] at-fault accident within the last three [3] years or;* any major violation (i.e. DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years.Other driving related violations may be considered unacceptable.
