Job summary

The IBHH Care Coordinator will join the Integrated Health Home team responsible for an individual’s total care, using a team-based approach to address physical, behavioral and social health needs with a single Health Action Plan (HAP).

Responsibilities include delivering tailored services via in-person and telephonic/electronic interventions, coordinating care with a multidisciplinary team, and ensuring engagement and activation of clients within a safety-sensitive, drug-free workplace.

Qualifications

• Minimum 21 years of age with AZ driver's license and clean driving record.
• Able to pass pre-employment drug screen and background check.
• Safety sensitive position under AZ Drug Testing of Employees Act.

The IBHH Care Coordinator will be a part of the Integrated Health Home (IBHH) Team which is accountable for an individual’s total care and will deploy a multidisciplinary team-based approach to address the physical, behavioral, and social health needs for attributed individuals, utilizing a single Health Action Plan (HAP). Services delivered by the IBHH Team are highly individualized, address complex and chronic conditions, and capitalize on available health information and actionable data through the use of technology and appropriate information sharing methods.
