Job summary

La Familia Health Santa Fe seeks a Behavioral Health Case Manager to coordinate care and services for patients with emotional and mental health needs under the Behavioral Health Director.

You will schedule appointments, provide targeted case management, assist with access to resources, and document treatment plans while coordinating with behavioral, psychiatric, and medical teams. Bilingual in English and Spanish and CPR/BLS certification preferred.

Qualifications

• Bachelor’s degree in social services, counseling, or social work.
• Two years of related field experience.
• Fluent in English and Spanish; bilingual required.
• CPR/BLS certification required or preferred.
• Certified Community Health Worker preferred.

Responsibilities

• Schedules patients and conducts reminder calls.
• Provides targeted case management to behavioral health patients.
• Coordinates care with behavioral, psychiatric, and medical providers.
• Develops and monitors care plans and treatment reporting.
• Assists with Medicaid/Medicare referrals and access to services.
• Maintains patient records and supports information sharing.
