Job summary

Elk Ridge Community Health in Flagstaff, AZ is seeking a Behavioral Health Case Manager to join the patient care team. The role focuses on direct case management, coordinating with clinicians, and supporting patients and families to improve treatment adherence and outcomes.

The position requires a Bachelor’s degree (or Master’s preferred) in a relevant field, 1 year of related experience, BLS certification, and a fingerprint clearance card.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology, Public Health or related field.
• One year of experience in human services, behavioral health, or case management.
• Current BLS certification for health care providers.
• Valid Level I fingerprint clearance card, or applied within seven working days of start.

Responsibilities

• Provide direct case management to patients enrolled in behavioral health services.
• Coordinate care with clinical team members to ensure quality services.
• Identify resources and services to support patients’ daily living and social determinants of health.
• Maintain HIPAA-compliant documentation and protect patient confidentiality.
• Initiate referrals and support transitions to needed services in the community or within the organization.
