Job summary

LaSante Health Center in Brooklyn is seeking a compassionate Behavioral Health Case Manager to support patients in our outpatient clinic. You will help navigate care, address barriers, and coordinate services to keep patients engaged in treatment.

The ideal candidate is proactive, personable, and comfortable serving underserved communities in a fast-paced setting. Experience in healthcare or behavioral health is preferred, with strong communication and organizational skills.

Qualifications

• Requires high school diploma or equivalent.
• Associate’s or bachelor’s degree preferred but not required.
• Experience in healthcare, behavioral health, outreach, care coordination, or customer service strongly preferred.
• Strong communication and organizational skills.
• Bilingual candidates encouraged to apply.
• Experience with Medicaid or underserved populations is a plus.

Responsibilities

• Assess patients for social, behavioral health, and practical support needs.
• Assist patients with accessing community resources and services.
• Coordinate care between providers and outside organizations.
• Conduct outreach calls to schedule, confirm, and follow up on appointments.
• Support patient engagement and retention in treatment.
• Document interactions and patient needs in the EMR.
• Work with therapists, psychiatrists, medical providers, and administrative staff.
• Assist with referrals, transportation coordination, insurance issues, and other patient support needs.
