Job summary

La Familia Medical Center in Santa Fe, NM is seeking a Community Care Coordinator to improve access to LFH medical, dental, behavioral, and support services for the homeless population. You will collaborate with community partners, coordinate health programming, and provide health education and navigation to patients.

The role emphasizes bilingual English/Spanish communication, Medicaid enrollment assistance, and data quality in reporting. A Bachelor’s degree or 4 years of healthcare exp.

Qualifications

• Bachelor’s degree preferred or four (4) years’ health care related experience, required.
• Prefer candidate with experience in community health, patient navigation, or care coordination.
• CPR Certification, required.
• Fluent in both English and Spanish, required.

Responsibilities

• Assist with improving awareness of and access to La Familia Health services for patients and target community members.
• Coordinate community health programming, including health education and social determinants of health.
• Provide health education and motivational interviewing to patients; identify barriers and navigate to LFH services.
• Assist with Medicaid enrollment, patient registration, and scheduling services in community health settings.
• Track and submit required patient and activity reports accurately and timely.
