Job summary

Pueblo Community Health Center, Inc. seeks a Behavioral Health Care Coordinator to support access to behavioral health services for its patient population. The coordinator manages incoming calls, BH triage, and schedules for BH therapy and psychiatric treatment.

The role generates reports, handles release of information requests, and coordinates BH provider schedules. A Bachelor’s degree with three years of related experience is required, with occasional local travel.

Qualifications

• Bachelor’s Degree with three years relevant experience in community primary health care required.
• Strong leadership, advocacy, and communication skills (written and verbal).
• Valid driver’s license and auto insurance as required by law.

Responsibilities

• Ensure the department functions align with Pueblo Community Health Center's mission and service model.
• Adhere to guidelines and procedures of Pueblo Community Health Center, Inc.
• Facilitate patient access to BH services, staff and resources.
• Manage BH provider schedules and ensure appointments are filled per protocol.
• Interact with patients who may be in emotional or mental crises and provide warm handoff to BH providers.
